Nominations are due by April 18, 2022, 5:00 p.m. CST. If you are nominating more than one person, you must submit a separate application for each nomination.
Criteria:
- Compassionate Care
- Places high-quality care to patients, their families, each other, and the community.
- Service Excellence
- Places focus on teamwork and the needs of their colleagues.
(Examples include where a colleague went above their job requirements to help a patient, a fellow nurse in need, or demonstrated their commitment to helping the community.)
